Irregular Periods, Infertility and PCOS

Ovarian cysts are sacs filled with fluid that are found in the ovaries.They are usually harmless and will disappear naturally on their own. However, polycystic ovary syndrome (PCOS) is a condition that affect a woman's health. It is one of the most common causes of irregular periods and infertility. 


Any woman can get PCOS and they usually become aware of it when they are trying to conceive. Some factors that contribute to PCOS include insulin resistance (which increases the risk of developing type 2 diabetes) and higher levels of male hormones.


When should you suspect you might have PCOS?


> Irregular or no menstrual periods, or abnormal spotting.

> Obesity where these women tend to gain weight even when they seem to be on a well-controlled diet.

> Infertility.

> Facial acne, excessive body hair and hair loss, usually caused by higher levels of male hormones.

> Acanthosis nigricans, which are patches of dark, velvety and thickened skin patches, located along the neck creases, underneath the breast or along the groin.


How is PCOS diagnosed?


The most commonly used method is the Rotterdam Criteria, which requires the woman to have at least 2 out of 3 of the below symptoms:


> Have very few periods or no periods at all.

> Have a polycystic ovary.

> Have high levels of male hormones.


What to do if you have PCOS?


PCOS is usually not life-threatening, but women with PCOS might be at risk of uterine cancer as the inner lining of their uterus can become very thick due to the non-shedding caused by irregular menstruation.


> Infertility

Some methods used: ovulation induced via medication, fertility treatments like IUI or IVF, laparoscopic ovarian drilling as a last resort.


> Excessive body hair

Hormonal treatment or physical treatment eg waxing, depilatories and electrolysis.


> Irregular menstrual period

How to Strengthen Bones and Reduce Risk of Breaking

Once women hit menopause, their lower oestrogen levels leave them more vulnerable to osteoporosis. It is a silent disease in the sense that there are usually no symptoms until the bone breaks. The spine, hips and wrists are especially prone to fractures, even if the person doesn't fall.


In people with osteoporosis, the bones can be so brittle that normal activities like lifting objects, bending over or coughing, can cause an unexpected fracture. Fractures can lead to chronic pain, deformities and disability, resulting in poor quality of life, dependance on others for daily activities and higher medical bills.


The best screening test to detect osteoporosis is dual energy x-ray absorptiometry scan (DEXA scan). It measures the density of bones (BMD). It is recommended that women aged above 65 and men above 70 go for a BMD measurement. Screening should be done earlier in patients with strong risk factors for developing osteoporosis, including:


(i) premature menopause,

(ii) being on prolonged steroid treatment, or

(iii) having anorexia nervosa and certain other medical conditions.


Some things you can do to prevent and reduce the risk of fractures include:


(i) Adequate calcium and vitamin D intake.

(ii) Measures to minimise the chances of falling.

(iii) Lead a healthy lifestyle by getting adequate exercise.

This article talks about the application of Shuang Hor products in addressing different kinds of diseases related to 3 main systems in our body - Immune system, Nervous system and Circulatory system.


1. Immune System

(a) Immune-Regulatory Effect

- Hardly have fever.

Old people have tens of thousands of cancer cells. One of the ways the body protects itself is to group all cancer cells together to form a tumour.
If size of tumour is less than 1 cm, it is undetectable.
If size of tumour is 3-4 cm, it can be detected. By then, it may be at stage 3, especially for colon cancer.

How Lingzhi works

For auto-immunity problems, Lingzhi suppresses the activity of T and B cells. At the same time, it boosts up the activity of NKC and phagocytes. YK2 works more effectively for those with auto-immune problem due to the higher content of triterpenoids.

Take at least 20 YK1 or 3 YK2 within an hour to suppress the activity of T cells.

1. Western Treatment

1(a) Medicine

These 4 methods are usually advised if the size of the growth is less than 5 cm.

a) Watchful waiting
Doctors may decide not to prescribe anything. If the person's liver function is strong, the growths may resolve on their own.

b) Using birth control pills or hormone therapy
The main function is to stop ovulation and prevent the developments of new cysts. It also slows endometrial tissue growth and prevent new implants of endometrial tissue. But it isn't a permanent fix for endometriosis.

c) Control and lessen symptoms uterine fibroids and polyps
The most commonly used drug is gonadotropin-releasing hormone (GnRH) agonists, which blocks the production of estrogen and progesterone. 
Another method is by using progestin-releasing intrauterine device (IUD) or Mirena to help relieve heavy bleeding.

d) Tranexamic acid (Lysteda)
It is used to treat heavy menstrual bleeding. 

Side effects of medicine

a) Birth control pills or hormone therapy
By using this method, high levels of estrogen will be produced, which convinces your pituitary gland that you are pregnant and you don't need to ovulate anymore.

Mild side effects: Larger breasts, weight gain or loss, reduced or increased acne.

b) GnRH agonists

1(b) Operation / Surgery

If the growth is bigger than 5 cm, doctors may recommend surgery.

a) Remove ovarian cysts
  • Laparoscopy (for small cysts). A surgical procedure where an instrument is inserted through the navel to perform minimally invasive surgery.
  • Laparotomy (for large cysts). A large incision is made on the abdominal wall and if the growth is found to be cancerous from biopsy, a hysterectomy (removal of ovaries and uterus) will be performed.
b) Remove uterine fibroids
  • Hysterectomy - removal of uterus.
  • Myomectomy - removal of fibroids only.
c) Remove uterine polyps
  • Hysteroscopy - removal of polyps via the insertion of a tube through the vagina. Biopsy is then conducted.
d) Remove endometriosis
  • The initial surgery will seek to remove only the endometriosis (laparoscopically or conventional abdominal surgery). 
  • hysterectomy and removal of both ovaries may be necessary for severe cases of endometriosis. The side effects include osteoporosis and accelerated ageing.
2. Naturopathy

Practicing a healthy lifestyle and diet helps prevent growths.

a) To relieve pelvic pain
  • Warm baths and a heating pad can help relax pelvic muscles, reduce cramping and pain.
  • Avoid cold drinks.
  • Regular exercise.
b) How to shrink fibroids and cysts naturally
  • Take complex carbo and low glycemic carbo to prevent diabetes (thus reducing risk of cysts) and prevent obesity (associated with development of fibroids). Avoid intake of white rice, white sugar and white bread.

Women's Diseases (Cysts, Fibroids, Polyps and Endometriosis) on Womb, Uterus and Ovary

This post gives you an overview of the 4 main diseases related to women's reproductive organs, namely cysts, fibroids, polyps and endometriosis.

Before that, let's take a look at the functions of 2 major hormones that can cause havoc here, which are estrogen and progesterone.

Functions of Estrogen

1) Send message to uterus to grow and replace the lining that's shed during previous menstruation.

2) Maintain proper rate of bone breakdown (osteoclast) and prevent bone loss (osteoporosis).

3) Develop female sex characteristics - breasts and sexual organ.

4) During pregnancy, reduce the function of thyroid hormone. This reduces metabolism rate, thereby conserving energy for the growth of foetus. As a result, ladies put on weight during pregnancy.

5) Increase production of thyroid binding globulin (TBG), which decreases metabolism and increases fat deposits to have enough energy for the foetus if a pregnancy develops.

6) During puberty and pregnancy, the secretion of estrogen is high but decreases after menopause. Excessive exercise can also lead to lower estrogen because women with too little fat cannot produce sufficient estrogen.

Too high or too low level of estrogen can lead to depression, anxiety and insomnia, decreased sex drive and concentration.

Functions of Progesterone

1) Produced after ovulation to maintain the stability of endometrium and provide the environment for embryo growth. If the lady is not pregnant, progesterone reduces and signals the beginning of menstruation.

2) Reduce response to estrogen receptor. For example, high estrogen secretion can cause breast enlargement; so progesterone appears to stop the continuous growth of breast cells.

3) Stimulate bone osteoblast (make new bones to replace old bones). If not, it increases the risk of osteoporosis.

4) Reduce TBG - stored fats are utilised. Low progesterone level can lead to weight gain. It's common for ladies to take hormone pills for their menstruation problem. But the side effect is, these drugs block the production of estrogen and progesterone thus causing the ladies to put on weight.

5) Stimulate sex drive as it can be converted to testosterone.

6) Affect the brain by reducing anxiety, insomnia and depression.


Now let's look at the different types of reproductive system diseases.

1. Ovarian Cysts


(a) Functional cysts

(i) Follicle cyst (also called water cyst)
During a woman's menstrual cycle, an egg grows in a sac called a follicle, which breaks open to release the egg. But if the follicle doesn't break open, the fluid inside the follicle can form a cyst. It's the most common type.

(ii) Corpus luteum cyst
Follicle sacs typically dissolve after releasing the egg. But if it doesn't and the opening of the follicle seals, additional fluid can develop inside the sac. This accumulation of fluid causes a corpus luteum cyst. They are usually harmless and disappear by themselves.

(b) Others
(i) Ovarian dermoid cysts
Sac-like growths that contain hair, fat and other tissues.

(ii) Ovarian cystadenomas
Non-cancerous growths on the outer surface of ovaries.

(iii) Ovarian endometriomas (also called chocolate cyst)
Tissues which normally grow inside the uterus develop outside and attach to ovaries (15% of cysts types).

(iv) Polycystic ovary syndrome

Cysts (i) and (ii) can become large, causing ovaries to move out of position, leading to pain.

Symptoms - pelvic pain, bloating, heaviness in abdomen.
Complications - twisting of ovary, rupture (causing internal bleeding), ovarian cancer especially higher risk for postmenopausal women.
Causes - functional cysts are due to menstrual cycle. The others are unknown.

2. Uterine fibroids (benign tumour)
(a) Intramural
Grows inside the wall of the womb. It's the most common.

(b) Submucosal
Grows towards the inside of the uterus cavity and hence will affect pregnancy.

(c) Subserosal
Grows towards the outside the uterus.

Symptoms - heavy menstrual bleeding, period lasting more than 1 week, pelvic pressure or pain.
Complications - anaemia from heavy blood loss, infertility, miscarriage.
Causes - genetic, imbalanced estrogen and progesterone hormones, insulin-like growth factor (diabetic patients are at higher risk).

3. Uterine polyps

They are growths attached to the inner wall of the uterus that extend to the uterine cavity. In other words, they are the overgrowth of cells in the lining of uterus (endometrium) which lead to the formation of uterine polyps (endometrial polyps).

They are usually benign although some can eventually turn cancerous.

Symptoms - vaginal bleeding, bleeding after intercourse, heavy menstrual bleeding.
Complications - infertility, miscarriage.
Causes - too much estrogen.
Risk factors - being peri or postmenopausal, has HBP, is obese or takes Tamoxifen drugs for breast cancer.

4. Endometriosis

Endometrial tissue normally grow in the uterus to prepare the lining of the womb for ovulation. Endometriosis is the buildup of these tissues that grow outside the uterus. These displaced tissues cannot be expelled during menstruation. 

It may effect the ovaries (more frequent), fallopian tubes, peritoneum and lymph nodes.

Symptoms - painful period, pain during intercourse, pain during bowel movement or urination.
Complications - infertility, ovarian cancer.
Causes - retrograde menstruation, transformation of peritoneal cells (due to hormones or immune factors), transformation of embryonic cells (due to estrogen), immune system disorder (not able to recognise and destroy endometrial tissues growing outside the uterus).

1. Do you know your hormones can make you fat?

Leptin is the hormone that controls your hunger pangs. It tells you your stomach is full so that you will stop eating.
Ghrelin is the hormone that tells you you're hungry; it's time to eat.
If these 2 hormones are not functioning properly, say Ghrelin keeps giving you hunger signals and Leptin doesn't tell you to stop eating, then you will overeat and put on weight.
What can you do?

It brings more stress to our body and increases the production of cortisol hormone (a stress hormone produced by the adrenalin gland). This blocks the progesterone receptor and leads to the reduction of progesterone being produced (and all the associated problems).

Eating unhealthy food can cause digestion problem.
Digestion problem can lead to hormone imbalance, as the balance of good and bad bacteria in your gut will be affected. The bad bacteria will lead to inflammation happening in your gut, which in turn will suppress your hypothalamus and pituitary gland to tell your body to produce less estrogen and progesterone, which will lead to adrenalin gland and thyroid gland also over/under produce hormones etc., which leads to further hormone problem.
It is a vicious cycle. Hormone problem aggravates the digestion situation, and digestion problem can cause hormone problems. Medication cannot address the root cause of this inter-related problem.

The word "douche" is French for wash or soak.
