Cortisone Steroids and Osteoporosis

Long-term treatment of rheumatic illnesses like arthritis using cortisone (a type of glucocorticoids) can cause osteoporosis. Cortisone is a steroid hormone and reduces allergic-like reactions. According to the German Society for Rheumatology (DGRh), as a preventive measure, you should accompany it with calcium and vitamin D

Experts say cortisone drugs are usually well tolerated when used for short periods. But if used for more than 3 months, there may be side effects like decrease in bone density, which lead to osteoporosis. In 30-40% of patients treated with cortisone over a period of 4.5 years, recent or older bone fractures could be detected, DGRh estimates. 


The problem is that many patients don't notice the small bone fractures precisely because of the medicine they are taking. Therefore, the bone problems are frequently overlooked and not treated. As a result, the skeleton becomes more fragile, thus promoting further fractures. In the first 3-6 months of glucocorticoid therapy, bone density decreases by up to 12%. 


After the end of treatment, the fracture risk declines again, as it is directly related to cortisone intake. Therefore, DGRh recommends taking as much cortisone as necessary, but as little as possible, in combination with preventive measures against possible bone loss.


In other words, a rheumatoid arthritis patient is faced with a serious challenge - they take cortisone to treat their arthritis. But long-term consumption may lead to brittle bones and osteoporosis.


What is a preferred alternative to cortisone?

Osteoporosis is a disease that causes bones to become weak and brittle. It occurs when the creation of new bone can't keep up with the loss of old bone. The risk of osteoporosis is higher in people who are frail, underweight, small in built and post-menopausal women. 


Many people think losing bone density and sustaining fractures are simply a natural part of the ageing process, but this is a Myth. Statistics show that 1 in 4 people who fall and fracture the hip will not survive beyond 12 months - which is worse than some cancer diagnoses! Unfortunately, despite its devastating effects, osteoporosis remains under-diagnosed and under-treated.


Factors affecting bone health include: 


  • Nutrition - nutritional malabsorption, eating disorders.
  • Age - don't take solid food, don't chew well.
  • Hormones - post-menopausal.
  • Lifestyle - physical inactivity.
  • Pre-existing diseases - diabetes, thyroid disorders, rheumatoid arthritis.

Hip Fractures and Osteoporosis

Bone fractures in the elderly are almost always caused by weak bones from osteoporosis or a fall. It is common that hip bones are the ones that are fractured. If someone has a hip fracture and wants to be pain-free, he would have to be bed-ridden for 3-6 months. The bone may heal but it could be in a displaced position and he would be unable to walk properly and fall again. Also, staying for prolonged duration in a lying position can cause lung complications, pneumonia, blood clots, bed sores, etc. 


For that reason, doctors prefer hip fractures in elderly to be operated on. Unfortunately, surgery has many risks due to their age. Putting them under anaesthesia is also risky. Further, the elderly may be unfit for surgery if they have blocks in the arteries and their cardiovascular risk for hip replacement is very high.


In such cases, doctors can only pump them with painkillers and blood thinners to prevent strokes or deep vein thrombosis. They would likely need nursing care. It is a financial and emotional strain to their family.


Those who can undergo surgery would have to sit up within 3-4 days and undergo chest rehabilitation to enable them to breath well. By day 5, they have to be transferred to a wheelchair. It would be a very painful process during these 3 months.

How to Strengthen Bones and Reduce Risk of Breaking

Once women hit menopause, their lower oestrogen levels leave them more vulnerable to osteoporosis. It is a silent disease in the sense that there are usually no symptoms until the bone breaks. The spine, hips and wrists are especially prone to fractures, even if the person doesn't fall.


In people with osteoporosis, the bones can be so brittle that normal activities like lifting objects, bending over or coughing, can cause an unexpected fracture. Fractures can lead to chronic pain, deformities and disability, resulting in poor quality of life, dependance on others for daily activities and higher medical bills.


The best screening test to detect osteoporosis is dual energy x-ray absorptiometry scan (DEXA scan). It measures the density of bones (BMD). It is recommended that women aged above 65 and men above 70 go for a BMD measurement. Screening should be done earlier in patients with strong risk factors for developing osteoporosis, including:


(i) premature menopause,

(ii) being on prolonged steroid treatment, or

(iii) having anorexia nervosa and certain other medical conditions.


Some things you can do to prevent and reduce the risk of fractures include:


(i) Adequate calcium and vitamin D intake.

(ii) Measures to minimise the chances of falling.

(iii) Lead a healthy lifestyle by getting adequate exercise.

(iv) Limit caffeine and alcohol intake and abstain from smoking.

Testimony: Osteoporosis Since Teenager

Ms I, 39, had osteoporosis since she was 15 years old. The score from her bone density x-ray was -3, which was very bad for a young person of her age. Doctor gave her Fosamax drug (which is usually only meant to be taken by the elderly). The side effects include stomach ulcer and cancer. She took the medication for 7 years. Her results grew from -3 to -1.8 (yearly increase of approximately -0.15). Doctor then stopped her medication since 2014 due to the side effects. There was no more medication prescribed.
Since she came to know about Shuang Hor products in 2018, she started taking Lingzhi + Jia Hor Soya Protein Yung Kien Pollen + 1-2 capsules Jia Hor Calcium per day.

Testimony: Back Pain

Ms E is 46 years old now. In 2009, when she was 36, she had a very bad car accident. But she didn't really bother and still proceeded with hill climbing the next day. One evening, I mis-stepped while walking down the stairs of a restaurant. I fell and sprained both my ankles. There was a sharp pain but I could walk to my car and quickly drove home. I did not apply any ointment or take anything else. The whole area was warm, sore and tender to the touch. The next morning, as expected, the inflammation got worse. I could barely put any weight on my left foot. I was wincing in pain. Thankfully the right ankle was slightly better. I had great difficulty walking down the stairs. I limped to the kitchen and had another 2 sachets Vitaking. 3 hours later I had another 1 sachet. Then 2 hours after that, I took another 1 sachet. The pain gradually eased. At night I took another 2 sachets.

The pain subsided so much that the next day, I recklessly overused them - I walked over 5,500 steps. Few days after that, I went to the gym. I continued with 4 sachets/day for 1 week then reduced to 2 sachets/day in the second week. I could walk, stand for hours and walk up stairs without problem except there was soreness when I walked down stairs. Due to all my activities, recovery took longer. But I am grateful that I do not need to curb my daily activities during this period.

Testimony: Hip Pain and Vitaking

Ms A attends yoga classes and heard a lady complain about having pain in her hip area. She has tried Western  and acupuncture treatment for a few months, but it doesn't seem to be getting better. 3 Effective Tips To Protect Your Bones

Our bones are living tissues, comprising mainly calcium and protein. If more bone calcium is being absorbed into the body than is replaced, the density (bone mass) is gradually reduced, the bone becomes weaker, increasing the risk that it may break. Osteoporosis develops when bone is no longer replaced as quickly as it is removed.

50% of women and 25% of men over age 50 will have an osteoporosis-related fracture. While gradual loss of bone mass is a fact of life for everyone, it's never too late to take steps that will protect our bones.

The amount of bone mass you obtain while you are young determines your skeletal health for the rest of your life. Meaning the more bone mass you have when you are young, the more protection you have against losing bone mass later.

Here are some tips to help children develop maximum bone mass, and help adults slowdown its loss.

1. Get adequate calcium and vitamin D

99% of the body's calcium is stored in the bones and teeth - a "bone bank". Calcium is "withdrawn" from the bone bank daily to support some vital body functions. If sufficient calcium is not "deposited" back via diet, the bones become deficient in calcium and eventually weaken.

The US Institute of Medicine recommends the following amounts of daily calcium from food and supplements:
  • 1 to 3 years old - 500mg
  • 4 to 8 years old - 800mg
  • 9 to 18 years old - 1,300mg
  • 19 to 50 years old 1,000mg
  • more than 51 years old - 1,200mg
Sources of calcium
Dairy products, almonds, broccoli, spinach, cooked kale, sardines, soy products eg. tofu.

Excessive consumption of calcium doesn't provide additional benefits and may even be harmful.

Vitamin D helps the body use calcium. The US National Institutes of Health recommends 400-600 IU (international units) of vitamin D for children and 600-800 IU for adults.

Sunlight, fortified milk, tuna, sardines, egg yolks.

2. Exercise

Both strength training and weight bearing exercises are important.
Strength training exercise strengthens muscles and bones in the arms and upper spine.
Weight bearing exercises (walking, jogging, running, stair-climbing) strengthens bones in the legs, hips and lower spine.
Cardiovascular workout (swimming, cycling) are less helpful for improving bone health than weight-bearing exercises.

3. Avoid smoking and regular alcohol consumption

The precise medical causes have not been identified, but researchers find that these 2 activities also contribute to weak bones.

Additional risk factors include family history, some medical conditions and certain medications.

To assess the health of your bones, you can take the bone density test known as DXA (dual energy x-ray absorptiometry). It takes 5 to 10 minutes and uses very little radiation. The T-score falls into 3 categories:
  • normal density
  • osteopenia (low density)
  • osteoporosis

New research findings are beginning to show that strong, healthy bones play a far greater role in our overall health than we previously realised.

What We Currently Know About Bone Functions

1) A fundamental role of our skeletal system is to provide structural support for our muscles and protection for our internal organs. It gives shape to our physique and allow us to be mobile.

New Insights

The osteoblast cells in our bones have been found to produce a hormone-like signaling protein called osteocalcin.

Leptin levels should be kept in balance as overly high leptin level can lead to leptin resistance. Leptin resistance (usually in obese people) causes their body to be unable to respond to leptin, so the brain cannot cue a person to stop eating, resulting in them putting on weight.
