The Importance of Adrenaline Hormones

Adrenal glands are located on top of our kidneys. They produce hormones such as:


(a) Cortisol - helps maintain normal blood pressure, glucose levels and sleep-wake rhythm.

(b) Aldosterone - stabilises blood pressure, salt and water in the body.

(c) Adrenaline (epinephrine) and noradrenaline (norepinephrine) - make the heart pump faster, regulate blood pressure and glucose levels.


The release of hormones by the adrenal glands is controlled in part by the pituitary gland. The kidneys and sympathetic nervous system also interact with the adrenal glands. These hormones are essential and the body cannot function without them. Disorders of the adrenal glands will result in the production of too much or too little hormones.


Causes of adrenal disorders include:


> Genetic mutations eg. congenital adrenal hyperplasia and adrenal leukodystrophy.

> Damage to adrenal glands through injury, infection or blood loss.

> Adrenal tumours.

> Prolonged use of steroids eg. prednisolone, hydrocortisone or dexamethasone, which may suppress the pituitary gland from producing ACTH (hormone that regulates adrenal function), thereby causing adrenal insufficiency. Or inversely, causing Cushing syndrome, a condition due to excess cortisol.


Many of the symptoms of adrenal gland disorders may be similar to other illnesses and thus are not easily identified.


Symptoms of adrenal insufficiency include:


> Unexplained poor weight gain.

> Easily tired.

> Severe illness.

> Low blood pressure and glucose levels.

> Salt cravings.

> Dark skin pigmentation.


Symptoms of excess production of cortisol in children include:


> Weight gain, more prominent at the face and belly.

> Poor growth despite weight gain.

> Muscle wasting.

> Increased hair growth over the body.

> Complications such as HBP, diabetes, prone to infections, easily bruised skin, brittle bones and irregular menses in adolescent girls.


It is hard for the body to modulate and balance the Hormonal system because:


> The hormones are released into blood vessels and circulated by blood flow throughout our body. So hormones affect nearly every process in our body. If the hormonal system is not regulated, it would affect our whole body's health.


> The Hormonal system coordinates with the Nervous and Immune systems, which are also spread throughout the whole body. Any disorders in any of the 3 systems will affect the overall functions of our body organs.

Endocrine System Diseases: Application of SH Products

When we talk about the metabolic and endocrine system, we need to touch on hormones, as they are related. Diseases in this system may not be fatal, but they can make you feel very uncomfortable.

The endocrine / hormonal system has various glands which produce and release hormones into the bloodstream, including pituitary gland, thyroid gland etc. Hormones control our daily activities, including our heartbeat, digestion, sugar level, stress level etc.

Kidney secretes a variety of hormones including erythropoietin, calcitriol and renin. People with kidney diseases will also have hormone imbalance. Because of that, people with kidney problem are more prone to have diabetes and vice versa, people with diabetes will have poor kidney function, as both are inter-related.

Liver also secretes many hormones (e.g. hormones that promote cell growth) and has many key functions associated with hormones in the body (e.g. regulating blood pressure or eliminating unneeded hormones). Liver malfunction affects hormone production. One of the raw materials of hormone is amino acid. Liver plays a function in converting protein into amino acid. If it's not functioning properly, it affects the conversion process.

Diseases are the end result. They are the manifestations of poor organ functions. Let's talk about the 3 main endocrine-related diseases, namely diabetes, ageing and kidney lesion.

1. Diabetes

What do the measurements of sugar level in blood (on empty stomach) mean?

- Less than 5.6 mmol/L (healthy)
- Between 5.6 - 6.9 mmol/L (pre-diabetic)
- More than 7.0 mmol/L (diabetic)

The conventional method of measuring one's sugar level is less accurate as it reflects reading at a particular point in time.

A more accurate measurement is HbA1c (in %), which is something that’s made when glucose (sugar) in your body enters RBC and binds to hemoglobin. Blood HbA1c levels are reflective of how well diabetes is controlled. It’s reflective of blood glucose levels over past 3 months and not daily ups and downs of blood glucose. However, note that the test may give misleading results in certain situations.

How do you interpret the readings?
- 4 - 5.6% (normal)
- 5.7 - 6.4% (pre-diabetic - higher chance of getting diabetes)
- More than 6.5% (diabetic)

RBC flow in our bloodstream. Carbo that is consumed will be converted into glucose, which also enters and moves in our bloodstream. Glucose (being a source of energy) needs to enter RBC so they can be carried into every cell in the body as nutrients for them.

(i) For glucose to enter RBC, it requires insulin. Pancreas is a gland that secretes insulin hormones to control blood sugar level in the body. Insulin acts like a key to bring glucose into RBC. If one does not have sufficient insulin, glucose is unable to go into RBC. Diabetes is a disease that happens when glucose cannot enter RBC i.e. excessive glucose is flowing in the bloodstream.

(ii) RBC receptors (which receive insulin) is defective or less sensitive. In such cases, although you have sufficient insulin, but due to poor receptor function, RBC is unable to allow insulin to enter.

Being diabetic is not fatal. It is the complications arising from diabetes that make it life-threatening, such as:
- heart failure or heart attack
- kidney failure
- stroke
- blindness
- amputation.

In general, the mechanisms of the drugs either:
(i) Stimulate the pancreas to produce more insulin. The side effect is the pancreas may be destroyed in the long-term.
(ii) Stimulate the sensitivity of the receptors so that RBC can receive more glucose.
(iii) Stop the function of liver to convert carbo to glucose. The carbo which cannot be converted ends up in the intestines. Side effects include diarrhoea and bloating.

Insulin is produced by pancreas.  What cause pancreas to malfunction?
(i) Bacterial / viral infections, causing pancreas cells to die.
(ii) Free radicals oxidise pancreas cells, thereby leading to malfunction of pancreas cells.

Western drugs are not able to revive or protect pancreas cells.

Combining Lingzhi with Jia Hor Fiber have the ability to delay conversion of starch to glucose and reduce the burden on the pancreas (as it is not being pressured to suddenly produce more insulin to handle the glucose). The glucose is converted gradually.

For patients with blood sugar reading of persistently above 10 mmol/L, it is recommended to take 2 sachets Fiber before each meal. This can help control the blood sugar level very well.

(c) Pollen

2. Ageing

Ageing is due to free radicals (FR). Every metabolic activity in our body, including breathing, creates free radicals. The more FR you have, the older you look. The reason is FR oxidise body cells and kill them, and so make you look older. FR is an unstable molecule. In order to stabilise itself, it needs to rob an electron from a body cell. The more FR there are, the more injuries to body cells.

Endocrine System and Hormones

The endocrine system is a collection of glands located throughout the body that produces hormones and release them into the bloodstream. Hormones govern our body functions, including growth and repair of tissues, sexual development and reproductive function, and the body's response to stress.

Primary functions of the endocrine system 

(a) Hypothalamus
It is located above the pituitary gland (at the lower central part of the brain). It secretes hormones that stimulate / suppress the release of hormones in the pituitary glands.

(b) Pituitary gland
It is often considered the most important part of the endocrine system because it produces hormones that control many functions of other endocrine glands.

(c) Pineal gland
Produces the melatonin hormone.

(d) Thyroid
Produces hormones that regulate body metabolism.

(e) Parathyroid
Secretes parathyroid hormones, which affect the calcium level in the blood.

(f) Thymus
Produces the hormone thymosin that stimulates the lymphoid organs to produce T-lymphocytes or antibodies in newborns and young children.

(g) Pancreas
Produces the insulin hormone and glucagon. Insulin regulates transportation of sugar, fatty acids and amino acids into body cells.

(h) Adrenal glands
Located on top of each kidney. The outer part (cortex) produces hormones called corticosteroids, which regulates metabolism and balance of salt and water in the body and secrete hormones that begin the development of secondary sexual characteristics. The inner part (medulla) produces hormones called catecholamines (e.g. adrenaline and cortisol) which help the body cope with physical and emotional stress by increasing the heart rate and blood pressure.

(i) Reproductive glands / gonads
Produce testosterone, oestrogen and progesterone.

(j) Prostaglandins
Known to decrease blood pressure, cause fever, inhibit hydrochloric acid secretion in the stomach, increase uterine contraction during pregnancy and influence intestinal peristalsis.

Disorders of the endocrine system

(a) Acromegaly
An enlargement of the bones of the hands, feet and jaws. This hormonal disorder happen when the pituitary gland produces too much growth hormone.

(b) Addison's disease
When the adrenal glands produce too little cortisol and aldosterone.

(c) Cretinism
Condition resulting from a congenital deficiency of thyroid secretion or hypothyroidism, resulting in severely stunted physical and mental growth.

(d) Dwarfism
Results from hyposecretion of the growth hormone of the pituitary gland, which has been caused by a tumour, infection, genetic factors or trauma.

(e) Gigantism
An excessive growth of the long bones caused by hyper secretion of the somatotropin hormone.

(f) Graves' disease
Caused by hyperthyroidism or thyrotoxicosis. Symptoms include abnormal protuberant eyes and swelling of the skin over the shins.

(g) Hyperparathyroidism
Causes hypercalcemia, an increased level of calcium in the blood.

(h) Hypoparathyroidism
A decreased secretion of parathyroid hormone that leads to abnormally low calcium levels in the  blood and bones. This can cause tetany (cramp like spasms of the hands and fingers).

(i) Hypoglycemia
Results from increased insulin production by the pancreas.

(j) Hypothyroidism
Resulting from an insufficient production of thyroxine, which can be caused by Hashimoto's disease.

(k) Syndrome of inappropriate antidiuretic hormone (SIADH)
A condition in which the body makes too much ADH. This hormone helps the kidneys control the amount of water the body loses through urine. SIADH causes the body to retain too much water. Thus it involves water intoxication and the dilution of intracellular and extracellular body tissues, usually resulting from lung cancer.

(l) Virilism
A condition in which women develop male-pattern hair growth and other masculine physical traits. Results from increased secretion of hormones in the adrenal glands.

Hyperthyroidism

Thyroid gland produces the hormone thyroxine which regulates the body's metabolism. Hyperthyroidism happens when the thyroid gland is overactive and produces too much thyroxine T3 and T4. Women are affected more than men.

Symptoms include anxiety, palpitations, weight loss, increased appetite, diarrhoea, menstrual disturbances and heat intolerance.

Management of hyperthyroidism

(a) Taking anti-thyroid drugs such as Carbimazole

The thyroid gland takes iodine and converts it into thyroid hormones T3 and T4. Iodine must be converted into a useable form before it can be combined with other components to form thyroid hormones.
Carbimazole works by preventing the conversion of iodine to its useable form and also stops converted iodine from being combined with other components to form thyroid hormones. This results in a decreased production of thyroid hormones. It may take 4-8 weeks for the levels to reduce to normal.

Side effects: Nausea, vomiting, skin rashes, headache, reduce white blood cells in rare cases.

(b) Taking Radio-iodine

It acts by destroying the thyroid gland.
Avoid during pregnancy and breastfeeding.
Patients with thyroid eye disease (TED) will become more severe.

(c) Surgery

This option is for patients with large goitre, particularly if it causes difficulty in swallowing or breathing.
Patients with bulging eyes may require corticosteroids as it does not respond to other treatments.
Have to take hormone pills for life and may affect the vocal cords.

Hypothyroidism

This is caused by too little thyroxine, and also due to immune system disorder. Babies with hypothyroidism can have permanent mental and physical retardation. Other causes include inborn enzyme disorder and deficiency of iodine in the diet.

Symptoms include weight gain, mental slowness, dry skin, hair loss, increased sensitivity to cold and heavy menstrual periods.

Traditional Chinese Medicine

From the TCM point of view, the Spleen controls the endocrine system and balance of hormones in our body. If the spleen is weakened, it will lead to hormone imbalance and endocrine system disorder. If the face looks yellowish, the spleen is weak. Other symptoms are indigestion, lethargy, the limbs lacking in strength and low appetite.

If one's spleen is functioning well, his hormones will function at optimum level too.

Testimony: Pregnancy, Pre-eclampsia and Lingzhi

When Ms M's cousin sister was 6 months pregnant with her first child, her blood pressure suddenly shot up to 200 mm/Hg. She was diagnosed as suffering from pre-eclampsia. The baby that was delivered was very small weighing only 1.6 kg and had to be put in an incubator for 1 month.

Testimony: Hormonal Imbalance

Ms LC, 50s, said that 5-6 years ago, she suddenly had red patches all over her neck. A few days later they disappeared then reappeared again, including on her face as well. Testimony: No Menstruation for 4 Months

Ms MY said that her menstrual flow stopped for 4 months after she underwent certain treatment. She was very worried.
