Bone Fractures and Osteoporosis

Osteoporosis is a disease that causes bones to become weak and brittle. It occurs when the creation of new bone can't keep up with the loss of old bone. The risk of osteoporosis is higher in people who are frail, underweight, small in built and post-menopausal women. 


Many people think losing bone density and sustaining fractures are simply a natural part of the ageing process, but this is a Myth. Statistics show that 1 in 4 people who fall and fracture the hip will not survive beyond 12 months - which is worse than some cancer diagnoses! Unfortunately, despite its devastating effects, osteoporosis remains under-diagnosed and under-treated.


Factors affecting bone health include: 


  • Nutrition - nutritional malabsorption, eating disorders.
  • Age - don't take solid food, don't chew well.
  • Hormones - post-menopausal.
  • Lifestyle - physical inactivity.
  • Pre-existing diseases - diabetes, thyroid disorders, rheumatoid arthritis.
  • Medications - steroids.

In osteoporosis, you can fracture the spine without having to fall. So if you bend over to pick up a pot of flowers or your grandchild, you may get back pain. This could be a sign of a fractured spine. It is essential to prevent even the first fracture, because once you have had one fracture, you're likely to experience another. There are medicines that can be tailored to suit all sorts of patients. However, for me, I won't take medicines to prevent osteoporosis.

Hyaluronic Injections Not Useful for Knee Osteoarthritis

Knee osteoarthritis is the deterioration of the cartilage in knee joints, leading to pain and difficulty walking. Injections of hyaluronic acid have been prescribed since the 1970s to treat this problem. Hyaluronic acid, a gel-like substance, is injected into the joint aiming to lubricate movement and decrease pain. The treatment is known as viscosupplementation.


A meta-analysis published in The BMJ looked at 169 previous trials that compared the injections to a placebo (no treatment). The researches then whittled that down to 24 large placebo-controlled trials involving nearly 9,000 patients, in what is the largest review of the available data so far.


These are some of the Conclusions:


> Strong conclusive evidence that viscosupplementation is associated with a clinically irrelevant reduction in pain intensity.

> The treatment is associated with a higher incidence of serious adverse events.

> It is not only ineffective compared with a placebo but might also be seriously harmful.


One of the study's authors, Bruno da Costa of the University of Toronto said that for a typical patient, it should be a last resort. Several large-scale, industry-funded studies were carried out that found negative results about the treatment. But those studies were never published, da Costa said. He added this is concerning, and perhaps mainly driven by commercial interests.

> France's social security system stopped offering reimbursements for the treatment a few years ago.

> England's National Institute for Health and Care Excellence recommends against viscosupplementation.

> A quarter of the money that US Medicare spent on viscosupplementation in 2018 went towards treating infections in joints following an injection, one of the most common side effects.

This post shares with you the ingredients and benefits of the 3 main building blocks our body needs - protein, collagen and calcium.

Hip Fractures and Osteoporosis

Bone fractures in the elderly are almost always caused by weak bones from osteoporosis or a fall. It is common that hip bones are the ones that are fractured. If someone has a hip fracture and wants to be pain-free, he would have to be bed-ridden for 3-6 months. The bone may heal but it could be in a displaced position and he would be unable to walk properly and fall again. Also, staying for prolonged duration in a lying position can cause lung complications, pneumonia, blood clots, bed sores, etc. 


For that reason, doctors prefer hip fractures in elderly to be operated on. Unfortunately, surgery has many risks due to their age. Putting them under anaesthesia is also risky. Further, the elderly may be unfit for surgery if they have blocks in the arteries and their cardiovascular risk for hip replacement is very high.


In such cases, doctors can only pump them with painkillers and blood thinners to prevent strokes or deep vein thrombosis. They would likely need nursing care. It is a financial and emotional strain to their family.

Difference between Sprain and Strain and How to Treat Them

Sprains and strains have similar symptoms but they affect different tissues in the body.


What are the difference?


Sprain - stretching or tearing of the ligaments (tissues that connect 2 bones). Common joints sprained are ankles, wrists, knees, hips or fingers.


Strain - injury to muscles or tendons (tissues that attach muscles to bones). Mostly occur in lower back and thigh muscles or hamstrings. Sometimes called pulled muscles.


What are the symptoms?


Sprain - pain, swelling, bruising and movement limitations. A "popping"sensation when you injured it.


Strain - pain, tenderness, redness, bruising, limited movement, muscle spasms and weakness.


As the symptoms are similar, the best guide is the location of injury.


Causes


Sprain - walk on uneven or slippery surface, land awkwardly after jumping (eg while playing basketball), during exercise, stopping a fall with your hands. More likely to sprain your joints when you are feeling tired or if you keep using old sports shoes which don't offer adequate protection.


Strain - lift something heavy without taking necessary precautions, repetitive movements eg typing on the keyboard or playing computer games using the mouse, playing contact sports like football and hockey, playing sports with a lot of quick starts, stops and jumps eg basketball, having higher risk of muscles strains in hand grip sports like golfing, straining the elbow in racquet sports like tennis or dart throwing.


Do you need to see a doctor?


Mild sprains - use ice pack several times a day to reduce swelling, elevate and rest the joint.

See a doctor if you cannot bear weight on the affected joint, have pain directly over the bones of that joint (it may mean a fracture) or have numbness in any part of that area.

Another Solution


I have sprained my ankles twice from falling down steps when walking and also while looking at my phone (I know, I know, I should have known better). My ankles were horribly swollen and bruised and I could barely put any weight on them. 


Instead of seeing an orthopaedic doctor or a sinseh or applying any cream, this is what I did.

Testimony: Osteoporosis Since Teenager

Ms I, 39, had osteoporosis since she was 15 years old. The score from her bone density x-ray was -3, which was very bad for a young person of her age. Doctor gave her Fosamax drug (which is usually only meant to be taken by the elderly). The side effects include stomach ulcer and cancer. She took the medication for 7 years. Her results grew from -3 to -1.8 (yearly increase of approximately -0.15). Doctor then stopped her medication since 2014 due to the side effects. There was no more medication prescribed.

Testimony: Back Pain

Ms E is 46 years old now. In 2009, when she was 36, she had a very bad car accident. But she didn't really bother and still proceeded with hill climbing the next day. One evening, I mis-stepped while walking down the stairs of a restaurant. I fell and sprained both my ankles. There was a sharp pain but I could walk to my car and quickly drove home. I did not apply any ointment or take anything else. The whole area was warm, sore and tender to the touch. The next morning, as expected, the inflammation got worse. I could barely put any weight on my left foot. I was wincing in pain. Thankfully the right ankle was slightly better. The pain gradually eased. At night I took another 2 sachets.

I could walk, stand for hours and walk up stairs without problem except there was soreness when I walked down stairs. Due to all my activities, recovery took longer. Uniqueness of Vitaking Collagen

One of the most unique feature about Vitaking is that it is hydrolysed. And there is no chemical substances and synthetic components in the ingredients.

(a) A lot of collagen products in the market are in big molecules and are not easily broken down by the body when consumed. As a result, it will be seen as a foreign substance by the body's immune system, which attacks it and triggers auto-immunity problem.

(b) Another problem of big molecules is the body cannot absorb thus whatever collagen that is consumed will be wasted.

From the diagram above, you can see the cartilage is located at the end of the bones, which are supported by ligaments. The cartilage consists of approximately 90% water and the balance 10% is made up of chondrocytes which comprise of collagen and proteoglycan.

Cartilage is manufactured by our own bones. The edge of the bones will grow the cartilage which then acts like a cushion between the 2 bones.

Before the age of 25, the production rate of collagen is higher than utilisation rate. This explains why young people don't experience knee pain and their skin look supple and bouncy.
After the age of 25, the production rate deteriorates at the rate of 1.5% every year, eventually resulting in deficit of collagen.
By the age of 40, you have depleted 25-30% of collagen in the body. That explains why many people start to have kneecap problem and find it difficult to walk.

Higher risk group include:
- People who are overweight because their excess weight burdens the knees more and thus use up more collagen.
- Badminton / basketball players as they use their knees a lot.
- Mountain hikers as the tendency to hurt their knees is higher.

This doesn't mean that you should avoid these activities. The only way to slow down this deficit process is to replenish with the right amount of collagen because it is a natural ageing process. Otherwise, undergoing a total kneecap replacement surgery is very costly, ranging from RM25,000 per leg to RM80,000 for both legs.

Glucosamine is a Common Solution

The most common solution  to treat joint pain problem is taking glucosamine.
Our bones synthesise cartilage. As we age, the rate of production becomes lower. How glucosamine works is by stimulating the growth of proteoglycan and chondrocytes by "pushing" them out of the bones.

Unfortunately, there is one negative consequence of long-term consumption of glucosamine that pharmacists usually don't highlight, which is how to consume it in the right way. It is advisable to stop for 1 month after every 3 months of consumption. If you continuously take, you will develop a disease which is triggered by over consumption of glucosamine, as the ingredients they are trying to stimulate i.e. proteoglycan and chondrocytes are over-produced. The kidney is unable to metabolise and flush them out quickly enough thus leading to the disease. Not only will it cause more burden to the kidney, it may even worsen the pain in the knee joints.

However, Vitaking works differently. As it is already hydrolysed, when you consume it with water, it goes directly to the area of inflammation. It doesn't trigger the growth of proteoglycan and chondrocytes but the collagen is directly provided to the area of inflammation so you won't experience excessive production of these 2 ingredients.

How to Consume Vitaking

We have 216 joints in our body. That's why for people who are already experiencing pain in their joints, they do not see much results by taking only 1 sachet Vitaking every 2-3 days. Over a short period, it is not enough to address the deficit problem in the cartilage.

Thus the right way to consume is to start with high dosage to address the problem. After you recover, you can reduce to maintenance dose. For example, people who experience sharp pain in their knees should start with 4 Vitaking a day. If it is very painful, take all 4 sachets at the same time, mixing with warm water (easier to dissolve and absorb). If you don't have financial issues, you can take it twice a day. Take in this manner for the first 5 boxes.

For the next 5 boxes, take 2 sachets / day if the pain has subsided. In other words, the patient should invest in 10 boxes of Vitaking to properly treat they problem. In fact, it works out to approximately Buy 9 Free 1, after discount.

For people with financial issues, they can buy 1 box at a time. However, they should be prepared to take up to about 10 boxes. Don't expect to see significant results after just 1 box.  When you supply sufficient raw material in one go, the recovery rate is faster compared to taking a little bit at a time. Testimony: Hip Fracture

Ms A's mother-in-law had a fall end Oct 2017, when she was 79 years old. She has been taking high blood pressure and high cholesterol medication for a long time. During the day she was not feeling so well, and had a bit of giddiness. That night, she miss-stepped, fell backward and hit her head on the floor. The family took her for an x-ray and the orthopedic said she had a hairline fracture on the left side of her pelvic bone and a little swell on her head. There was no concussion even though she felt a lot of pain.

As it was at night, there was nothing much the doctor could do. She was only given painkiller. As she had been on long-term high blood pressure medication, her blood pressure shot up during the incident to 180 over 100. The doctor gave her some injection to control the blood pressure. She couldn't walk and stand, and was in a lot of pain. The painkiller in fact was not enough to give her the relief. For the first week, she was fed 1 capsule Yung Kien Ganoderma2 + 4 capsules Yung Kien Ganoderma + 2 sachets Vitaking each time, twice a day. She took this dose for one week. She couldn't take her bath and couldn't sleep well.
Healing reaction: indigestion, heartburn, diarrhoea and felt very tired (but couldn't sleep). Her frequent trips to the toilet is good as it helps flush the inflammation out of her body.

On week 2, she reduced the dose to 3 YK1 + 1 Vitaking (whereby the YK is diluted in Vitaking as she had difficulty swallowing capsules) + 3 Lactoberry + Jia Hor Calcium each time, twice a day.
She still had similar healing reactions but the severity was reduced. She could sleep longer at night and the pain was more bearable.

Healing reaction: a lot of phlegm. She could walk with a cane and the pain was much more bearable.

On week 4, she was able to stand longer.

The orthopedic said she would need 100 days to recover from the hairline fracture. But her healing was very fast. She recovered within 2 months. By Christmas, she was able to stand, cook, bathe and do the normal activities. She was very happy that the discomfort was only for a short period of time. Difference between Glucosamine/Chondroitin v. Collagen

Cartilage is the connective tissue found in many parts of the body, including the knees. In a simple explanation, cartilage (comprising collagen and proteoglycans) is produced by the bones. As we age, even more so above 40 years old, lesser cartilage tissue is produced, and together with wear and tear, there will be lesser cushion amongst the bones, causing pain and inflammation. In other words, if the raw material is already low, and yet the bone is still forced to produce cartilage, the bone would really suffer.
